Reach Out and Read - an evidence-based model shown to build early literacy skills. Medical providers educate parents on the importance of reading to their children at home and give parents the means to do so, by providing them with a brand-new book to take home.
The high quality of HealthLinc’s clinical care, its providers and support teams is evident by its achievement of NCQA Level 3 PCMH for all clinics and receiving 100% compliance in its most recent HRSA certification survey. HealthLinc also boasts Family Nurse Practitioner and Clinical Pharmacy residency programs and a certified Medical Assistant Training Program. HealthLinc is committed to strengthening its clinical, support and administrative infrastructure to promote continued innovation and growth.
The Role: Reporting to the Chief Executive Officer, the Chief Quality & Informatics Officer will direct, administer, coordinate, and integrate the systems, activities and initiatives that drive the achievement of quality outcomes for our patients and provide timely, actionable information to support our providers and organization in achieving organizational goals and objectives. You will lead HealthLinc's Quality Management System to ensure organization-wide consistency and program and service excellence. You will analyze, design, develop, implement, and evaluate information and communication systems that enhance individual and population health outcomes, improve patient care, and strengthen the clinician-patient relationship. You will also develop and recommend administrative, governance and programmatic policies and procedures to coordinate functions between programs and services and help ensure compliance with accreditation, licensure, and other standards. You will serve as a member of the Leadership Team. In partnership with Team members, you will develop and implement plans to strategically position HealthLinc to be prepared for the transition from fee-for-service to a value-based payment system. Your success will rely on your own performance and your ability to develop and nurture collaborative relationships.
